Privacy, Transcription, and Appointment Records Notice:
The Social Deep, LLC / Seasons Change treats client information with care, discretion, and respect. Appointments may be recorded, transcribed, summarized, and documented for continuity of service, accuracy, appointment insight development, quality review, and mutual accountability/protection.
Transcripts, appointment insights, notes, forms, and related records are maintained as private appointment records. Appointment insights may be provided after services when appropriate, and transcripts may be available to the client upon request, subject to privacy, safety, legal, business, and recordkeeping limitations.
Client information and appointment records will not be intentionally shared with outside parties unless required or permitted by law, needed for safety or legal reasons, necessary for limited business operations, or authorized by the client in writing.
The Social Deep, LLC / Seasons Change provides specialized, non-clinical, non-medical support. Services are not therapy, counseling, medical care, hospice care, legal advice, emergency response, or crisis intervention. Electronic communication and virtual appointments may carry privacy risks.
By booking and participating in services, the client acknowledges this notice and understands that additional privacy, consent, payment, and service terms may be included in separate written forms or agreements.
